If a surgeon sees a patient in the hospital for a consult, and the patient then sees the surgeon at the office to further treat the problem, is that a new patient or established? We're torn here...

Established. Doesn't matter if you have no paperwork or not. Your doctor has treated the patient, and fairly recently at that, apparently. You don't get to charge a new patient office visit.Sharon
